      PDAs should make sure that they verify the validity of account details to which payments are to be made.

    What this means in practice

    Before the first distribution, the PDA must confirm the consumer's debit-order mandate and each creditor's bank account against the COB or written confirmation. A misallocation here recurs every month until corrected.

    Where it goes wrong

    Wrong creditor account numbers cause payments to land in the wrong bucket, which the credit provider then reports as non-payment. The fix is to pull the PDA distribution register and the credit provider's allocation report and reconcile them line by line.
