    Documents Required for Your Initial Visit

    Gather these documents before applying for debt counselling

    Your Documents

    • Copy of your identity document/passport
    • Most recent salary/wage slip (last 6 months if income varies)
    • Last 3 months bank statements for all accounts
    • Last 2 months credit card statements
    • Latest home loan balance statement (if applicable)
    • List of informal debts (from family/friends) (if applicable)
    • Most recent creditor statements

    If Married in Community of Property

    If you and your spouse share income and expenses or are married in Community of Property, you'll also need:

    • Spouse's most recent salary/wage slip (last 6 months if variable)
    • Spouse's bank statements for all accounts
    • Spouse's credit card statements

    Additional documents required when your matter proceeds to court (per NCR Circular 001/2016)

    For Court Application

    Additional documents required when your matter goes to court (per NCR Circular 001/2016):

    • Original or certified copy of Identity Document
    • Proof of residence (address within court jurisdiction)
    • Consumer's Confirmatory Affidavit (signed and commissioned)
    • Proof of Marital Status (Marriage Certificate, Antenuptial Agreement, or Divorce Order)
    • All payslips since application date
    • Form 16 (signed original)

    Documents required for consent order applications to the National Consumer Tribunal (per NCT Circular 7)

    For NCT Application (Section 138)

    Documents required for consent order applications to the National Consumer Tribunal (per NCT Circular 7 of 2018):

    • (1) Consent Order - attached separately, not stapled to main application
    • (2) Proof of Service - email to all credit providers with 15 business day wait
    • (3) Copy of Identity Document - clear and legible
    • (4) Power of Attorney Form - signed by consumer(s), specific debt counsellor named
    • (5) Acceptance Letters - signed by each credit provider in same order as consent order
    • (6) Draft Consent Order - signed by consumer(s) or debt counsellor with POA
    • (7) Repayment Proposal - signed and dated Form 16 sent to credit providers
    • (8) Income and Expenditure Statement - consumer's financial position
    • (9) NCT Application Form (Section 138) - fully completed, dated and signed
    • (10) Consent to E-mail Service Letters - from each credit provider
    • (11) Proof of NCT Filing Fee Payment (R717.79) - EFT to NCT with consumer names and ID numbers
