Standard Documentation — Overview
Annexure E is the Standard Documentation pack that accompanies the Debt Review Task Team Agreements. It collects the prescribed forms, the consensus letter templates and the field glossary every NCR-registered debt counselling system must use.
The pack covers: Form COB (Certificate of Balance), Form 17.1 (Notification), Form 17.2 (Proposal), Form 17.3 (Termination), the Standard Output Forms used by debt counselling systems, the consensus Letter of Acceptance and Letter of Decline templates, and a Glossary of Terms that defines every field used on those outputs.
Pages 67–78 of the source PDF are scanned images. The forms (pages 68–73) are reproduced here as OCR text, readable but with some artefacts in table cells. The glossary entries (pages 74–78) are cleaner. For absolute fidelity, refer to the original PDF.

The standard documentation set covers every party-to-party communication from Form 16 through to the consent order. Using the standard forms keeps the file admissible in court and aligned with creditor expectations.
