Standard Output Forms — Summary & Field Reference
The Standard Output Forms are the system-generated documents that every NCR-registered debt counselling system must produce. They standardise the way debt counsellors, credit providers and PDAs exchange information about an affordability assessment, a restructuring proposal, and the resulting payment plan.
The Glossary of Terms (clause E.8) defines every field used on these standard output forms. Annexure C sets out the system requirements every debt restructuring system must meet to produce these outputs reliably.
Field-reference summary (PDF pages 72–73, the source pages are large reference tables; the printable visuals live in the original PDF). The reference defines:
Document set: Certificate of Balance (COB), Debt Restructuring Proposal, PDA Payment Plan Summary, Assessment Summary.
Common header fields used across every output: Reference Number (unique case number), Date generated, NCR registration numbers for both DC and PDA, Juristic person registration number, full names, ID numbers and full contact details for the Debt Counsellor, the PDA, the Credit Provider and the Consumer.
Per-account fields: Account Description (as held on the credit provider's system, e.g. Revolving, Mortgage, Account), Linked Insurance policy description, NCA Account Type Code (see Table 1 below), Debt Restructuring Account Type Code (see Table 2 below), Goods Description (for Vehicle and Asset Finance), Inception Date of loan / facility / policy, Expiry Date of loan / facility, Original Repayment Term, Outstanding Balance at date of default (this field is not populated by the banks), Credit Limit (for credit cards and current accounts), Outstanding Balance at date of COB (incl. balloon payment and arrears, excl. future interest, incl. interest accrued but not yet levied).
Per-account amounts: Gross Monthly Instalment (incl. capital, interest, fees and insurance), Monthly Service Fees (as per NCA, excl. transactional or non-NCA fees), Method of Payment (debit order / EFT / cash), Included or Excluded from Debt Review, Reason Code where excluded (e.g. Juristic / Legal), Name of the bank where the credit provider is not itself a bank.
Payment instructions: Account name into which the PDA payment must be made, Reference number for payment identification, Suspense account tracking reference, Email address of the credit provider, Fax number with area code.
Table 1 — NCA Account Type Codes (selected): Microlender (ML); Bank — Credit Card (BCC); Bank — Bond / Mortgage / Home Loan (BMG); Bank — Overdraft (BOD); Bank — Vehicle (BVF); Bank — Loan (BL); Retailer — Furniture (RF); Retailer — Clothing (RC); Retailer — Other (RO); Legal Firm / Collections (LFC); Unsecured Term Loan (UT); Short-term loan / personal loan (incidental credit agreement with a defined term).
Table 2 — Debt Restructuring Account Type Codes (selected): SOV (Secured — Private Vehicle), SCV (Secured — Commercial Vehicle, taxi), Mortgage Bond, Unsecured Term Loan, Revolving Facility (overdraft, credit card, incidental credit agreement with no defined term).
